Jack Woodruff Slade

Jack Woodruff Slade, passed away Saturday, October 15, 2021 at the age of 55 in Cleveland, Tennessee from complications due to heart disease, renal failure and diabetes.

Jack was born August 27, 1966 in Van Nuys, California. Soon after his birth, his parents moved him to Eagar, Arizona to join his older brothers, Harvey & Ray and his older sister, Donna. He grew up roaming the 40 acres that included their home, fields and pastures.

He loved being outdoors and proved to be a good hand from an early age; doing farm chores and helping his dad with his excavation business. He attended Round Valley High School and enjoyed playing football.

In January of 1988 he married Dina Chavez and they had two children together, Calcee and Jensen.

Jack had a natural ability with all types of heavy equipment. As an adult he worked at various jobs; including excavation work and truck driving.

He was preceded in death by his parents, Jack and Zelda Slade and a sister-in-law, Deann Slade.

He is survived by his brothers, Harvey Slade, Ray Slade, sister, Donna (Don) Goodman, son, Calcee Slade, daughter, Jensen Slade, grandchildren, Caiden, Ezekiel, Olivia and numerous nieces and nephews.

A memorial celebration of Jack’s life will be held at the Eagar, Arizona Cemetery, Saturday, October 15, 2022 at 11:00 a.m.
